Records Verification

Official verification of academic records for graduates and students.

The Registrar's Office handles all requests for the verification of student academic records and credentials. This service is available to employers, educational institutions, and other third parties requiring official confirmation of enrollment or graduation.

How to Request Verification

  1. 1. Prepare Your Request Email

    Draft an email to records@mihp.edu.ph. To avoid delays, please include all items listed in the "Required Information" section below, especially the student's full name and signed consent.

  2. 2. Pay the Verification Fee

    A non-refundable processing fee of ₱200.00 is required for each verification request. Please attach the proof of payment to your email.

  3. 3. Send Your Request

    Send your email with all required information and proof of payment to the Registrar's Office at records@mihp.edu.ph.

Required Information

To avoid delays, please ensure your request includes:

  • Student's Full Legal Name (while enrolled)
  • Date of Birth
  • Program of Study / Degree Earned
  • Year of Graduation or Dates of Attendance
  • Signed Student Consent Form
